Upon receipt of your consent, we will notify you each time we prepare an eStatement or eNotice for an account that you have selected. We will send you an email letting you know that the information is available. Simply log into your account on Internet Banking, and click on the eStatement link. You agree it is your sole responsibility to keep your Internet Banking login information from unauthorized persons. You agree that the Credit Union has no control as to the persons who have access to your personal computer and your password once it is in your possession. The Credit Union will not be liable for any unauthorized access to your personal computer or your passwords.

By accepting the terms of this agreement, you hereby authorize UnitedOne Credit Union to provide notification of periodic account statements and notices to you by electronic mail. Your authorization means that we can provide you with this information on your account electronically. You are responsible for notifying us of any email address changes. If your electronic mail is returned as undeliverable, a paper copy of your statement or notice will be sent by U.S. mail and the “Return Mail Fee”, which is referenced in our Fee Schedule, will be charged to your account.

Your email address may be changed within internet banking, or by contacting one of our offices. We must receive notice of email change within 3 business days of a statement being generated by the Credit Union. The Credit Union will have no obligation or liability to any of the parties to a Multiple Party account if the email address is changed by one of the procedures above.

You have the right to withdraw your consent to receive electronic periodic eStatements or eNotices at any time. To withdraw your consent, please contact us at mail@unitedone.org, in writing at any of our offices, by phone at 920-684-0361 or 920-451-8222, or by stopping in to any of our offices and meeting with a Member Service Representative.

All electronic statements and notices shall be in full compliance with applicable laws and regulations. The provisions in this agreement are part of (and in supplement to) UnitedOne Credit Union’s Terms and Conditions and are subject to all provisions in the Terms and Conditions. Each UnitedOne Credit Union account that you designate to be included within the eStatement and eNotice program is also governed by the terms and conditions otherwise applicable to that kind of account as separately disclosed to you, either in the Terms and Conditions, applications and enrollment forms, the applicable personal or business fee schedules, credit or deposit agreements, our Privacy Policy or other written disclosures.

I have read and agree to the terms of the eStatement and eNotice Disclosure and I would like to receive eStatement and eNotice delivery. I understand that for the accounts I have signed up to receive eStatements and eNotices under, I will no longer receive a periodic statement sent by U.S. Mail.
